The six-day course registration fee is US$1,250 which includes up to 45.50 hours of CME, P.A.C.E.®, or Florida CLS credit, daily breakfast, lunch, snacks during breaks, a course reception with dinner, and a $100.00 non-refundable administrative fee.

Course materials are based on the Atlas of Clinical Fungi by Sybren de Hoog et al, which can be purchased at https://www.atlasclinicalfungi.org/ and includes an electronic searchable version.

Please bring your laptop and a lab-coat.

This is a live, in-person only course. No virtual option will be available.

**REFUNDS will be given to anyone who cannot attend for any reason so long as we receive notice of CANCELLATION FOUR WEEKS PRIOR TO THE COURSE. If you need to cancel your registration, please send an email to Alyssa Peña at alyssa.pena@aruplab.com with "REFUND: FDLC/ATLAS Course in Clinical Fungi" in the subject line. Please provide your contact information and invoice # referenced in the “REGISTRATION PENDING” email you received following your registration and will also be listed on the credit card payment confirmation email you received after submitting payment.

Conference Cancellation Policy:
The University of Utah Department of Pathology and ARUP Laboratories reserves the right to cancel or postpone this conference due to unforeseen circumstances. In the unlikely event of a cancellation or postponement of this conference, registration fees will be refunded in full; however, the University of Utah Department of Pathology and/or ARUP Laboratories is not responsible for any other related costs, charges, or expenses incurred by registrants of this conference.
