HIV Diagnosis
This lecture reviews the epidemiology as well as the pathophysiological progression of HIV infection. It further explores the evolution of clinical assays for HIV diagnosis, tracing their development from the early stages of the HIV epidemic to current testing approaches and diagnostic algorithms. Light is also shed on testing and diagnosis of HIV infection in special populations such as neonates. The lecture concludes by examining HIV therapy and management strategies, encompassing aspects such as pre- and postexposure prophylaxis.

Transfusion Reactions with Respiratory Distress
Respiratory transfusion reactions occur less frequently than allergic or febrile nonhemolytic transfusion reactions; however, they account for the highest number of transfusion-related fatalities, as actively reported to the FDA in the United States. They are also likely occurring at a rate of 5-10 times more frequently than are currently being passively reported. Careful assessment of patients before, during, and after transfusion is key to mitigating and treating respiratory transfusion reactions. This case-based educational review of transfusion-associated circulatory overload (TACO) and transfusion-related acute lung injury (TRALI) covers the basics of these entities with a practical approach to clinical understanding.

A manual examination of cellular morphology on a stained peripheral blood smear is still an important, and increasingly "lost" skill, useful in the evaluation of patients with hematologic abnormalities. Autoimmune Hemolytic Anemia
An 86 year-old male, O+, with a history of anemia presented with extreme fatigue in 2020 and significant weight loss. BMBx found erythroid megloblasoid maturation and abnormal cytogenetics. Diagnosed with low-grade Myelodysplastic syndrome. Severe anemia a year later, repeated BMBx and did not met MDS criteria. Post-Artesunate Delayed Hemolysis
A 61 year-old male with HTN transferred from an OSH with MET, acidosis, AKI, and hypoglycemia in the setting of severe malaria and an incidental COVID infection 5 days after returning from Mazambique. No malaria prophylaxis taken. Smear showed Plasmodium falciparum, parasitemia, 24.4%, for which he was started on Artesunate q12h x3 doses, then daily until parasitemia was <1% followed by coatem BID x3 days. Post coatem, he began to feel more fatigued with intermittent mild fevers and darkening urine. He became HDS and aferbrile, but his CBC was notable for a Hgb of 5.1 with PLT of 363. This presentation will further explore this case along with discussing post-artesunate delayed hemolysis in greater detail.

Providing a More Comprehensive and Personalized Approach to Genetic Disorders through Next-Generation Sequencing
The commercial introduction of next-generation sequencing (NGS) in 2005 ushered in a new biomedical research era by virtue of the technology’s ability to yield unprecedented DNA sequencing throughputs in a rapid and cost-effective manner. Eight years later, NGS is being increasingly applied as a molecular diagnostic tool in fields as diverse as oncology, infectious diseases, and inherited (genetic) disorders. NGS is transforming the diagnostic evaluation of genetic disorders by allowing for more comprehensive, personalized approaches. For example, multi-gene panels are being employed in a variety of genetic disorders wherein mutations in any one of a multiplicity of genes can result in overlapping signs and symptoms, and exome sequencing is being leveraged to identify causal and candidate genes in patients and families with undiagnosed disorders with genetic etiologies.

Genetic Susceptibility to Infectious Diseases
Most immunodeficiencies leading to increased susceptibility to infections seen in clinical practice are secondary to other diseases, such as infections (e.g. HIV) and malignancies (e.g. Multiple Myeloma), or are iatrogenic (e.g. immunosuppressive drugs). Primary immunodeficiency diseases (PIDD) are thought to be genetic in origin. There are now over 150 PIDD recognized, and their number is still growing. Most of these diseases are individually rare, but as group they have an estimated prevalence of 1:1200 in the USA, similar to acute leukemias. Diagnosis of PIDDs requires integration of data from clinical findings with laboratory immunological analyses and increasingly with genetic testing. In this presentation we discuss the utility of molecular diagnosis in immunodeficiency diseases, and then review specific examples of genetic susceptibility to bacterial, fungal and viral infections. The Lewis System
The Lewis system is unique among blood group systems in that the antigens are not manufactured within the erythrocyte, nor do they form an integral part of the cytoskeletal membrane. While antibodies against antigens in this system are fairly commonly encountered, they are generally not considered to be clinically significant in transfusion. Because Lewis phenotype expression is based upon the interaction of several genes, and because the phenotype expression can be transient, the Lewis system is a unique system to learn about.

Testing a Test: Beyond Sensitivity and Specificity
This lecture covers performance evaluation of diagnostic tests. Traditional performance measures such as sensitivity, specificity, and ROC curves are reviewed. Reasons for differences in diagnostic studies are examined including real differences, threshold effects, sources of bias, and random variation. Shortcomings of the traditional approaches to test evaluation are also discussed and alternative approaches such as diagnostic research (vs test research), clinical trial evaluation, and cost-effectiveness evaluation are presented.
